How to add comment box in article in our website

Hi All,
Didn't find the solution in the earlier post, so posting it again.

In my website, there are several articles. I want user to comment on my articles. For this i want to use the comment box, something like the image shown in below comment.

or please see the below link and check at the bottom of the page for the comment module. I am using simply ASP.NET. I want exactly like this.Please help.

Hi Ashutosh,
I can not see in your screen shot properly.

If you want comment box on your article than you can set textboxes and one post button.
When user fill Name,Email,Subject and message after comment will be for moderation under admin if you want moderate comment than you can allow directly on article.

It is very simple module for any article forum or blogging.

Hai Ashutosh,
I think you can add a user control after making it as per your need and then add it for each of the answer.
You can have a Multiview where on top part you can show the question and answers and then to the below view,you can have this user control.
While loading of the user control you can get the name of the user and place it to the box.
