Hi,

Project document consists of every single detail about the project.
Starting from describing the project, to feasible studies, requirement analysis, database design, data flow diagram, flow charts, coding standard, and so on.

As far as responsibility of a team leader is concerned, well it can vary organization to organization. But mainly it consists of:

Understanding the project by reading project document (sometimes also involved in making the document)
Divide the projects into modules and distribute among his/her team
Ensure the company standard while developing the project
Take care about the dead line of the project
Helping his/her team whenever required
Communicate with other TLs for example TL of designers, and other team involved in the project etc.

These are only general responsibility which I am sharing with you for an idea. The actual roles and responsibility varies from organization to organization.

Hope it'll help you.
Regards
Ajatshatru
