If you want to add it dynamically for the selected items you need to add create dynamic controls and add it.
Adding two times. where you need to add ?

hi,
Actually i have some offers say 5 offers and if someone need 2 items out of this 5 offers and he submits,there appears a form to fill.but since the user selects 2 offers,he need to fill 2 forms.the form content is all same except the logo.can anyone provide me the code?
